Details
This insert gives you total control over what information you’d like to record and remember. When opened in Adobe Acrobat Reader (a free download*), you will be able to choose from 33 different prompts from drop down menus. If you’d prefer to use your own categories, you can write them in. You can also delete the text on a line and leave it completely blank if you’d like more writing room for a certain prompt.

TIP: If your editable file prints with arrows showing, export it as an image (.JPG) in Adobe Reader and it will print perfectly.
Files
Once you download your files, you will see that they contain an underscore with a suffix at the end (e.g., "Filename_No Crop"). This chart will help you know exactly which files you will receive and how to identify them.
